"In Fight Club Edward Norton was supposed to throw a stage punch in the first fight outside the bar. The director pulled Norton aside and told him to actually hit Brad Pitt. The reaction from Pitt of getting hit is real and if you look closely Norton almost starts laughing from what he did. At the end of Die Hard Alan Rickman gets dropped off the building. Rickman was told he would be dropped on the count of 'three'. The director quietly told the rest of the crew to drop him on the count of 'two'. The footage you see is of Rickman almost pissing himself." Source

Dolph was reluctant but Stallone kept insisting. Stallone got hit and immediately crumpled to floor and had to rushed to a hospital were he stayed in intensive care for four days due to swelling around his heart." Source

"Mark Hamill was incredibly lonely during the filming of The Empire Strikes back. He has just a handful of mostly brief scenes with other actual human actors. In fact, I think you can count them pretty easily: Recovering from the Wampa attack on Hoth, The attack on the AT-AT walkers (barely counts; he had a gunner behind him, briefly), The fight with Vader on Bespin, Recovering post-fight on the rebel ship with Leia and the droids. Virtually every other scene he has in that movie he was interacting with either puppets (Yoda, R2D2), people in full body suits (Wampa), or special effects (spirit of Ben Kenobi). He related that during the Dagobah scenes, he would be on the large swamp set and Frank Oz was obviously below him controlling Yoda. Whenever they took a break, Frank would remove himself from Yoda and the puppet would unceremoniously slump over, dead looking. The crew also somehow seemed to vanish almost immediately, so he was left in the middle of this huge swamp set all by himself, feeling forgotten. Every time I watch Empire now, I think about poor lonely Mark Hamill." Source

"In the scenes of Lord of the RIngs when Legolas, Aragon and Gimlie are running across Rohan the actors had a cracked rib, broken toes and a knee injury(dislocation i think)(in that order), after each take they would double over in pain. A large portion of the Rohan Riders were women. The guy who plays Gimlie was allergic to the prosthetics on his face, but soldiered on for the whole filming. He also had no issue with hitting the stuntmen with his axe, so the fighting scenes with him are him swinging his axe half blind thanks to swelling and trying to actually hit people. IIRC the very last scene filmed with all the cast is the final scene where they all say goodbye, Peter Jackson kept on retaking it because he didn't want it to be over and by the end of it all the actors had tears in their eyes. The day that they filmed a scene with Golem trying to catch fish in a river, the entire place was snowed over, they had to use the film lights and heaters to defrost it all, the actor was leaping around the river in literally ice cold water. The actor who played Saruman wanted to play Gandalf his whole life but they were afraid he would die before the filming finished etc. so they cast him as Saruman. All the different races in the armies were played by different groups of stun men, who developed rivalries between the races. In once scene when the elves are shooting down at the orcs, the elves weren't getting into it so the enemy stuntmen started taunting them, they got so into it they starting taunting back and they had to cut."
